Submit a private membership application online. All information is held in strict confidence. Our practice manager responds within one business day.
Step II
A private consultation with your physician at 910 Park Avenue to understand your history, your goals, and to ensure the relationship is right for both parties.
Step III
Select your membership tier. Annual membership activates immediately. Your introductory assessment is scheduled within the first week.
Step IV
Your physician is available from day one. Your personal care protocol is established within the first 30 days of membership.
